The ultimate goal is to support the systematic adoption, implementation, and evaluation of successful programs, practices, and policy changes. The database provides carefully reviewed, documented, and ranked practices that range from good ideas to evidence-based practices.

The Rhode Island process builds on a recent agreement among all the New England Governors and Eastern Canadian Premiers to reduce greenhouse gases in the region to 1990 levels by 2010, 10% below those levels in 2020, and by as much as 75% over the longer-term.
The goal of the program was to recognize and address the diversity of local health disparities by marshaling local community involvement in the place-based Health Equity Zones.
The framework established through the Health Equity Zones allows for the continued collaboration between governmental public health entities and stakeholders in the community to address health disparities.

The goal of this program is to improve colorectal cancer screening rates among older adults.
Participants in the intervention group had significantly higher colorectal cancer screening attendance, as well as having more positive attitudes about screening and placing a higher priority on screening.

The Working toward Wellness program succeeded in increasing the likelihood that depressed Medicaid parents sought mental health services, particularly among the Hispanic population.
